Health Status after Invasive or Conservative Care in Coronary and Advanced Kidney Disease
Abstract
The ISCHEMIA-CKD trial evaluated health-related quality of life in patients with stable ischemic heart disease, moderate or severe ischemia, and advanced chronic kidney disease randomized to an invasive or conservative strategy. Nearly half had no angina at baseline. At 3 months, invasive care led to a modest improvement in angina-related health status, especially in those with frequent symptoms. However, by 6 to 12 months, differences between groups diminished. There was no substantial or sustained benefit from the invasive strategy across the full population, suggesting that conservative care may be equally effective for most patients in this high-risk cohort.
