Pelvic Splenosis

Abstract

This clinical image presents a rare case of pelvic splenosis in a 34 year old woman with a history of partial splenectomy following trauma. The patient reported chronic pelvic pain, and MRI revealed solid nodules with high T2 weighted signal intensity. Laparoscopic resection identified hypervascular lesions on the mesometrium and mesorectum (Panel A), confirmed as ectopic splenic tissue on pathological examination (Panel B). The condition, resulting from splenic tissue autotransplantation post trauma, can mimic gynecologic neoplasms. The patient’s symptoms resolved postoperatively. This case underscores pelvic splenosis as a differential diagnosis for pelvic pain in patients with prior splenic injury or surgery.
