Pituitary Apoplexy
Abstract
An 81 year old man with a history of a nonfunctioning pituitary macroadenoma presented with sudden-onset headache, vision loss, and right oculomotor nerve palsy. Imaging revealed hemorrhage within the pituitary tumor, confirming pituitary apoplexy a condition characterized by acute bleeding or infarction of the pituitary gland or tumor. The patient exhibited central hypothyroidism and adrenal insufficiency. Emergency endoscopic endonasal resection was performed, followed by hydrocortisone and hormone replacement therapy. By postoperative day 4, his vision returned to baseline, and the oculomotor palsy resolved within 3 months.
