Schistosomiasis

Abstract

This clinical image report describes a 13-year-old boy from Mali who presented with a 3-month history of gross hematuria, absent fever or dysuria. Lab tests showed elevated eosinophils (2660/mm³) and IgE levels (3544 IU/mL), with negative urine culture. Microscopy revealed parasite eggs with terminal spines, identifying Schistosoma haematobium. Cystoscopy showed polypoid bladder masses, and biopsy confirmed schistosome eggs and chronic cystitis without dysplasia. A circulating antigen test was positive. After praziquantel treatment, hematuria resolved and schistosome eggs cleared within one month. The case illustrates classic urinary schistosomiasis with strong diagnostic imaging and histopathology findings
