One Size Fits … Some

Authors:
Danielle Chammas, M.D.

Abstract

This article examines the challenges of applying standardized communication techniques in medical education and palliative care. The author reflects on a family meeting where a trainee struggled to connect with grieving relatives, despite following prescribed communication tools.

The piece critiques the one-size-fits-all approach in medical training, highlighting that communication strategies must be flexible to accommodate cultural expectations, emotional states, and individual needs. Instead of relying solely on scripted responses, clinicians should assess each situation carefully and adjust their communication style dynamically.

The author introduces the concept of "formulation," borrowed from psychotherapy, which encourages clinicians to continuously refine their understanding of a patient’s psychological and emotional state. By moving beyond rigid templates, medical professionals can cultivate trust, improve patient interactions, and provide better emotional support.

Ultimately, the article calls for more nuanced training in medical education, ensuring future doctors develop the ability to adapt their approach to real-world complexities rather than simply following structured guidelines.
