Pragmatic Trial of Hospitalization Rate in Chronic Kidney Disease
Abstract
This pragmatic trial evaluated the effectiveness of an electronic health record (EHR) based algorithm combined with practice facilitators in reducing hospitalization rates among patients with chronic kidney disease, type 2 diabetes, and hypertension. The study involved 11,182 patients across 141 primary care clinics, randomized into intervention and usual care groups. At 1 year, the hospitalization rate was 20.7% in the intervention group and 21.1% in the usual care group, showing no significant difference (P=0.58). Secondary outcomes, including emergency department visits, cardiovascular events, and mortality, were also similar between groups. The intervention did not improve disease control or reduce hospitalizations compared to usual care.
