Cytomegalovirus Retinitis
Abstract
This clinical image case describes a 34-year-old woman with HIV infection who developed a blind spot in her right eye. She was previously treated for tuberculous lymphadenitis and was on antiretroviral therapy. Her low CD4 count and recent prednisone use for immune reconstitution syndrome likely contributed to opportunistic infection. Funduscopy revealed fulminant retinitis with retinal necrosis and hemorrhage in both eyes. Cytomegalovirus (CMV) was confirmed via anterior chamber paracentesis. Despite treatment with intravenous foscarnet and intravitreal ganciclovir, the patient developed bilateral retinal detachment. Surgical repair stabilized her vision. This case highlights CMV retinitis in advanced immunosuppression.
