Chylothorax
Abstract
This clinical image report describes a 63 year old man with a history of follicular lymphoma who presented with fatigue and dyspnea. Imaging revealed bilateral pleural effusions and diffuse lymphadenopathy. Thoracentesis yielded 1 liter of milky fluid, and analysis confirmed chylothorax with elevated triglycerides and lymphocytic exudate. Cytology matched the prior lymphoma profile, indicating relapse. The patient responded initially to chemotherapy but relapsed within a month. Salvage therapy and CAR T cell treatment are planned. Chylothorax is most commonly caused by lymphoma among nontraumatic etiologies.
