Abdominopelvic Actinomycosis

Abstract

A 54 year old woman presented with a 2 month history of fevers, weight loss, and abdominal pain, along with difficulty walking. Clinical examination revealed a tender abdominal mass, and imaging showed an intrauterine device (IUD) surrounded by multiple abscesses extending into the iliopsoas muscle and hip joint. Gram staining and culture confirmed Actinomyces israelii infection. The IUD, which had been in place for over two decades, was removed, and the patient was treated with prolonged antibiotic therapy, leading to resolution of symptoms. This case highlights the rare but serious complication of abdominopelvic actinomycosis associated with long-term IUD use.
