Omental Endometriosis

Abstract

This clinical image depicts a rare case of omental endometriosis in a 37 year old woman with a history of ovarian endometriosis. The patient presented with cyclical lower abdominal pain during menstruation. MRI revealed a 4.1 cm cystic mass in the lower abdomen with high T1 weighted signal intensity (Panel A). Laparoscopic resection confirmed an omental origin (Panel B), with pathological analysis confirming endometriosis. The patient’s symptoms resolved postoperatively, and she achieved pregnancy 15 months later. This case highlights extrapelvic endometriosis as a differential diagnosis for abdominal pain in patients with a history of endometriosis.
