Case 27-2023: A 53-Year-Old Woman with Celiac Disease and Upper Gastrointestinal Bleeding
Abstract
This case involves a 53-year-old woman with celiac disease who developed progressive watery diarrhea that became bloody, resulting in hemorrhagic shock due to upper gastrointestinal bleeding. Investigations revealed portal hypertension, hepatic steatosis, severe nutritional deficiencies, and multiple complications including bleeding esophageal varices, a retroperitoneal paraganglioma, and hepatic schwannoma. She was diagnosed with Turner syndrome based on physical features and karyotyping. A transjugular intrahepatic portosystemic shunt (TIPS) and liver biopsy confirmed noncirrhotic portal hypertension (portosinusoidal vascular disorder). Management involved intensive care, endoscopic interventions, and multidisciplinary evaluation.
