Managing Stable Ischemic Heart Disease
Abstract
This editorial examines insights from the ISCHEMIA and ISCHEMIA-CKD trials regarding the management of stable ischemic heart disease, comparing an initial conservative approach with an invasive strategy involving angiography and potential revascularization. The authors highlight that both strategies yielded similar rates of major cardiovascular events, with invasive care offering greater relief from angina for symptomatic patients. In patients with chronic kidney disease, the invasive approach conferred no benefit and showed potential risks. The piece underscores the importance of guideline-based pharmacologic therapy and shared decision-making when tailoring treatment strategies.
