Abstract

This Images in Clinical Medicine entry reports a rare case of complex umbilical cord knotting in monochorionic, monoamniotic twins delivered via cesarean section at 32 weeks of gestation. Antenatal monitoring revealed severe variable decelerations in one twin, prompting intervention. Both twins were born with Apgar scores of 7 (1 minute) and 8 (5 minutes), and cord blood gas analysis was normal. The umbilical cords exhibited intricate knots, a rare finding associated with intrapartum complications and fetal distress. The twins were monitored in the neonatal intensive care unit and discharged after 40 days without sequelae.
