Twin Anemia Polycythemia Sequence
Abstract
This Images in Clinical Medicine entry describes a case of twin anemia polycythemia sequence (TAPS) in monochorionic, diamniotic twins born at 31 weeks of gestation. The first twin exhibited pallor and anemia (hemoglobin 6.9 g/dL), while the second twin showed plethoric appearance and polycythemia (hemoglobin 25.5 g/dL). Placental analysis revealed asymmetric vascular changes, corroborating the diagnosis. Initial ultrasonography suggested TAPS, but subsequent findings indicated progression to twin to twin transfusion syndrome (TTTS). The anemic twin received a packed red cell transfusion, and the polycythemic twin underwent phototherapy and exchange transfusions. Both twins achieved normal development by 12 months.
